Kdump Failling with MPTdriver
Issue
- On an IBM iDataplex kdump is failing with the following error:

mptsas: probe of 0000:04:00.0 failed with error -16
- The kdump code then hangs waiting for the disk devices to appear.

Waiting for 1 sda-like device(s)...
Environment
- Red Hat Enterprise Linux 6.3
- LSI Logic / Symbios Logic SAS1064ET PCI-Express Fusion-MPT SAS (with firmware version older than 1.30.10.00)
