Upgrade to RHEL 5.8 and later 2.6.18-308.* kernels the server fails to boot

Issue

  • upgrading/installing new kernels (RHEL5.8) 2.6.18.308.11, 2.6.308.13, or 2.6.18-308.16 won't boot
  • existing kernel 2.6.18.8.1.el5 boots ok
  • appears to be missing cciss driver in initrd for Smart Array P420
  • cciss driver appears to be missing from later kernels
  • Host keep crashing and rebooting after OS upgrade

Environment

  • Red Hat Enterprise Linux 5.8
  • HP Smart Array P420
  • HP DL380 Gen8 server
