Horizon does not allow to add/remove/modify metadata

Issue

• Update metadata dialog field is empty
• Unable to add, remove, or modify metadata on flavours, images, or volumes in Horizon
• In a newly installed OpenStack environment, selecting dropdown for admin/flavors and clicking on update metadata gives an empty window.
