Why does RHEL6 fail to load qla2xxx module with error message "qla2xxx [xx:xx:xx]-x:x: Failed to load firmware image (ql2500_fw.bin)"?

Issue

• The firmware of a Qlogic Fibre Channel Card is not loaded and below errors are reported in dmesg file and slows down the booting process.
