RHEV storage domain extension executed twice resulting in an inactive storage domain and extraneous "unknown device" PVs

Issue

The sequence of UpdateStorageDomainCommand/ConnectAllHostsToLunCommand/ExtendSANStorageDomainCommand was executed twice concurrently by RHEV-M for the same request to extend a storage domain.
As a result, the block-based Storage Domain ended up with extraneous unknown device PVs, which caused the Storage Domain to become inactive.
