Red Hat Cluster Suite node is shutting down instead of rebooting with power fencing

Issue

Cluster node is shutting off but should reboot.
When fencing a cluster node using power fencing, the node turns off but not back on.
fence_ilo fails to reboot node.
fence_wti powers off a node, but does not turn it back on.
fence_bladecenter powers off a node, but does not turn it back on.

Environment

Red Hat Enterprise Linux 4 with Red Hat Cluster Suite
Red Hat Enterprise Linux (RHEL) 5 Update 4 with High Availability Add-Ons
cman-2.0.115-1.el5 or older
If using HP Integrated Lights-Out (iLO) 2, iLO2 firmware version 1.70 or later.
