XFS filesystem becomes corrupted and enters read-only state with: "XFS internal error xfs_da_do_buf(2)"
Issue
- The filesystem will periodically become corrupted and enter a read-only state with the following error visible in
/var/log/messages:

Environment
- Red Hat Enterprise Linux 5.5+
- kernel-2.6.18-194.el5 or greater
