How do I recreate the CloudForms workers on RHEV-M

Issue

We have CloudForms and I had 2 RHEV CloudForms workers on my rhev-m server. The Fiber channel storage got hosed and I need to rebuild these workers. Can you provide me with the step by step procedures to recreate the worker appliances?
