How do I generate an A4-sized PostScript printer test page in RHEL 6?

Issue

On RHEL 5 we can then print a CUPS test page to verify a print queue in fact does work correctly:

lpr  -P print-queue /usr/share/cups/data/testprint.ps

However, on RHEL 6 we have a few issues with this test page. First, there simply is no postscript test file available anymore. There does exist a file /usr/share/cups/data/testprint but it contains only some text content. We have found that this new testprint file can be processed by the command bannertops, but this brings us to the second problem: the generated postscript file turns out to be a letter-sized page, not A4. Even if we explicitly add a page size, the generated postscript still appears to be letter-sized.
