Why did my LVM devices go offline after losing some paths to storage?

Issue

• There was a storage failure and some paths for my multipath devices went offline. Even though not all paths were lost I got messages like these in the logs:

  kernel: ext3_abort called.
  kernel: EXT3-fs error (device dm-9): ext3_journal_start_sb: Detected aborted journal
  kernel: Remounting filesystem read-only
  

• Read-only filesystem on failure of some of the storage paths
