After a mirror leg fails, the mirror LV is not converted to linear LV in RHEL 6

Issue

  • After a storage failure, a complete site went down. This affected one mirror leg in a number of mirrored devices. Even after there were no remaining paths in the multipath device of the mirror leg, there was no change to linear.
  • My cluster experienced a site failure with one storage array going down, but rgmanager resources became blocked or failed to stop

Environment

  • Red Hat Enterprise Linux (RHEL) 6
  • lvm2
    • Using releases prior to lvm2-2.02.104, OR
    • In/etc/lvm/lvm.confdevices { ignore_lvm_mirrors = 0 }` or is unspecified
