System crashed with message kernel BUG at drivers/scsi/qla2xxx/qla_os.c:657!
Issue

During I/O from a host with single FC port to a storage system with three FC ports, one connection was shut down from the system side. After continuing to run for a while with the two remaining connections, the host module crashed with following error messages on console:
