System crashed with message kernel BUG at drivers/scsi/qla2xxx/qla_os.c:657!

Issue

  • During I/O from a host with single FC port to a storage system with three FC ports, one connection was shut down from the system side. After continuing to run for a while with the two remaining connections, the host module crashed with following error messages on console:

     sd 8:0:0:2: rejecting I/O to offline device

     qla2xxx [0000:04:00.1]-8803:8: Abort command mbx failed cmd=ffff8803be510700, rval=101.
     qla2xxx [0000:04:00.1]-3815:8: SP reference-count to ZERO -- sp=ffff8803be40f340 cmd=ffff8803be510700.
