Server kernel panic volume group not found

Issue

• We rebooted our prod server which was up for sometime now. After reboot, the kernel panic'ed saying that the volume group is not found. We had to boot to a older kernel version.
  After it came up, tried to recreate the initrd but system still failed to boot with kernel panic.
