fence_apc reports "Unable to connect/login to fencing device" and stonith devices won't start and/or report errors in pcs output in a RHEL 6 or 7 High Availability cluster

Issue

There are numerous "Unable to connect/login" errors in my logs from fence_apc

Dec 04 01:28:04 node1.example.com fence_apc[12762]: Unable to connect/login to fencing device

fence_apc sometimes fails and sometimes succeeds when I run it manually, reporting "Unable to connect/login to fencing device" when it fails.
My APC stonith devices sometimes don't work.

Environment

Red Hat Enterprise Linux (RHEL) 6 or 7 with the High Availability Add On
pacemaker
fence_apc in use as the stonith device agent for one or more nodes
