RHEL6: Oops from kernel NULL pointer dereference in ext4_clear_inode()

Issue

  • Server crashed with a kernel panic from an kernel NULL pointer dereference in ext4_clear_inode()
  • prior to the panic, we see VFS: Busy inodes after unmount of dm-286. Self-destruct in 5 seconds. Have a nice day...
  • An HA failover was being performed using 3rd-party clustering software. During the attempted failover, the cluster package providing an NFS export was started/stopped multiple times unsuccessfully. Finally the admin began doing "umount -l" to unmount the filesystems.
