Server is missing LVM mount points

Issue

  • The system had lvm volumes configured over SAN devices, recently the LVM mount points are missing in lvm command outputs:

    [root@host1 ~]# lvscan
  No volume groups found

[root@host1 ~]# pvscan

  No matching physical volumes found

[root@host1 ~]# vgscan
  Reading all physical volumes.  This may take a while...
  No volume groups found

  • How to recover the previously created lvm volumes? please provide RCA about why the lvm mount points are missing?

Environment

  • Red Hat Enterprise Linux 6.2
  • Logical Volume Manager 2 (LVM2)
