SCSI error with Additional Sense Code "Data Phase Error"

Issue

  • /var/log/messages reports errors similar to;

    kernel: SCSI error : <1 0 0 4> return code = 0x8000002
kernel: Current sdr: sense key Aborted Command
kernel:     Add. Sense: Data Phase Error

  • Path failures are logged around same time as the Data Phase Error logs.
