LVM commands hang in a cluster with "unable to obtain global lock" in RHEL 5 or 6

- LVM commands are hanging in a cluster stating its unable to obtain a global lock
$ pvdisplay
clvmd not running on node node1.example.com
Unable to obtain global lock.
Environment
- Red Hat Enterprise Linux (RHEL) with the Resilient Storage Add On
lvm2-cluster(clvmd)
