When trying to configure manual proxy settings in RHEL 6 KDE, username and password boxes stay greyed out

Issue

When using a proxy to access the internet, and configuring the options using the GUI and selecting "System Settings" --> "Network Settings" --> "Proxy" --> "Manual proxy configuration" the username and password boxes are greyed out and unable to be configured.
