System panic: lpfc crash in lpfc_device_reset_handler from NULL pnode
Issue
We would like you to assist in diagnosing a system panic which happened today on one of our production systems called hostname. The partner node took over services correctly so the customer impact was minimal. Unfortunately there is no system dump. The system came back without problems and was joined back to the cluster already. Last messages in syslog are:

Those lpfc ADISC error messages are known to us. They are all related to tape issues we have currently and are visible in syslogs of many RHEL5 and RHEL6 systems.
They are only reported for adapter ports 0000:05:00.0 and 0000:05:00.1 which are dedicated to tape access.
For disk access we have other 4 dedicated FC adapter ports on different HBAs.
There are no other messages in syslog nor ILO logs. The only other hint we have is a screen capture of the console which I will attach to this record.
