Fibre Channel paths dropped, need issue_lip to restore them due to regression in RHEL 5

Solution Verified - Updated -

Issue

  • systems are randomly dropping fibre channel paths to storage resulting in DID_NO_CONNECT
  • loosing san paths multiple times and having to manually recover them
  • paths easily restored by manually issuing a LIP
  • SAN devices under multipath are going down and devices returning error of:
Result: hostbyte=DID_NO_CONNECT driverbyte=DRIVER_OK,SUGGEST_OK

Environment

  • Red Hat Enterprise Linux 5.8
  • Red Hat Enterprise Linux 5.9 (prior to 2.6.18-384.27.1.el5)
  • Red Hat Enterprise Linux 5.10 (prior to 2.6.18-371.9.1.el5)
  • lpfc FC driver
  • Hitachi or HP Open-V storage (rebranded Hitachi)
