Issue

systems are randomly dropping fibre channel paths to storage resulting in DID_NO_CONNECT
loosing san paths multiple times and having to manually recover them
paths easily restored by manually issuing a LIP
SAN devices under multipath are going down and devices returning error of:

Result: hostbyte=DID_NO_CONNECT driverbyte=DRIVER_OK,SUGGEST_OK
