HBA card is showing offline with "Adapter failed init".
Issue
- HBA card is showing offline in one of the Redhat server.
- Seeing the following:
lpfc 0000:10:00.0: 0:0448 Adapter failed init, mbxCmd x8d READ_SPARM mbxStatus x0
- Need to verify the logs and find out whether issue is from OS side or hardware related.

- Red Hat Enterprise Linux 4
- Emulex LPe1105-HP FC Mezz HBA
