Login with correct password fails even though account is active

Issue

• SSH and/or console login fails for user account even when correct password is entered.
• The user account is not locked, disabled or expired.

Issue 1
Login is denied with the following error in /var/log/secure:

May 22 17:43:51 host0 sshd[14202]: error: PAM: Authentication failure for testuser from host0.testdomain.com
....
May 22 17:44:33 host0 sshd[14202]: Failed password for testuser from 10.xx.yy.zz port 43596 ssh2

Issue 2
Login is denied with the following error in /var/log/secure:

May 22 16:54:48 host0 sshd[12798]: pam_access(sshd:account): access denied for user `testuser' from `host0.testdomain.com'
May 22 16:54:48 host0 sshd[12784]: error: PAM: User account has expired for testuser from host0.testdomain.com
May 22 16:54:48 host0 sshd[12784]: fatal: monitor_read: unpermitted request 104
