Cluster is quorate but quorum disk offline
Issue
- quorum disk is offline
- disk is available to both nodes
- clustat shows the quorum drive as offline

errors like the following in /var/log/messages during cluster formation:
qdiskd[5357]: <warning> Minimum score (2) is impossible to achieve (heuristic total = 1)
Environment
- Red Hat Enterprise Linux Server 5 (with the High Availability Add on)
- Red Hat Enterprise Linux Server 6 (with the High Availability Add on)
