lpfc "SCSI layer issued Device Reset" messages in RHEL

Issue

  • Seeing SCSI errors in logs or on console:

    kernel: lpfc 0000:10:00.0: 0:(0):0713 SCSI layer issued Device Reset (0, 17) return x2002 
kernel: lpfc 0000:10:00.0: 0:(0):0713 SCSI layer issued Device Reset (3, 4) return x2002

  • Storage devices and/or paths are failing after seeing lpfc Device Reset messages.

Environment

  • Red Hat Enterprise Linux(RHEL)
  • Emulex HBA using lpfc device driver
