Trying to re-provision existing hosts is failing with 404 errors

Issue

Try to re-install a previously build system with the same boot image or pxe provisioning.

The error messages that can be seen for bootdisk:

no such file or directory http://ipxe.org/2d0c613b  
!PXE structure was not found in UNDI driver code segment

OR

404 errors like below for PXE:

GET /unattended/provision?token=8cc70a17-f408-4907-bf5c-85d7224dcc49 HTTP/1.1" 404 100
