After running mkinitrd with a LVM root disk listed in fstab to mount by LABEL=/, Red Hat Enterprise Linux 4 system fails to boot

Solution Verified - Updated -

Issue

  • Ran mkinitrd to have 'lpfc_max_luns' take affect. (Note: mkinitrd had not been run prior to this)
  • System Fails to boot with the following error reported on the console:
    Loading ext3.ko module
    Creating root device
    Mounting root filesystem
    mount: error 6 mounting ext3
    mount: error 2 mounting none
    Switching to new root
    switchroot: mount failed: 22
    umount /initrd/dev failed: 2
    Kernel panic - not syncing: Attempted to kill init!
     <0>Rebooting in 20 seconds..
    

Environment

  • Red Hat Enterprise Linux 4.8

  • Root disk is a LVM logical volume, and fstab entry lists the root volume as LABEL=/.
