Frequent failure of multipath causing readonly filesystem
Issue

Multipath fails frequently causing filesystems to be mounted read-only. Error messages in /var/log/messages as below.
end_request: I/O error, dev sdbm, sector 4192702 end_request: I/O error, dev sdbm, sector 4192708 device-mapper: dm-multipath: Failing path 68:0.
Environment
- Red Hat Enterprise Linux Server 5.3
- device-mapper-multipath-0.4.7-23.el5-x86_64
- HP blades (model Proliant BL460c G1)
- HDS AMS1000 storage system
