Provision Dialog Error trying to Create new Catalog Item

Issue

  • When we are trying to create new catalog item to provision new server, getting error "Cannot create Request Info, error: Dialog cannot be found. Name:[miq_provision_dialogs_template_Selfservice_Admin] Type:[MiqProvisionWorkflow]" displayed in the UI
