RHEL6.8+ kernels are exposed to panics if eh_deadline is enabled on adapters with SLI3 chip sets

Issue

Broadcom removed the host reset handler from the SLI3 host template in RHEL6.8+ kernels.
However a missing commit still allows the eh_deadline parameter to be set.
If the SAN has issues and the adapter enters recovery, the missing function can lead to unpredictable behaviour and system panics.

Environment

Red Hat Enterprise Linux 6.8+ kernels
EMulex adapters running the lpfc driver and the the SLI3 chip set.
