multipath SCSI device not created when scanning devices on RHEL 5

Issue

  • Multipath device is not created

  • Sometimes multipathd does not create multipath device with the following messages

    Jan 01 00:00:00 hostname1 multipathd: sda: add path (uevent)
Jan 01 00:00:00 hostname1 multipathd: mpath1: failed in domap for addition of new path sda

  • Multipath device is not created when HP StorageWorks Replication Solutions Manager executes the command multipath -v0.

Environment

  • Red Hat Enterprise Linux 5.3
  • Package earlier than device-mapper-multipath-0.4.7-30.el5
