Error "Adapter exlusively used by another host" while trying to add a qeth device in s390

Issue

• After booting zlinux onto a LPAR, qeth adaptor shows offline or DOWN. The errors in the logs are:

qeth: loading qeth S/390 OSA-Express driver
qeth: IDX_ACTIVATE on read channel device 0.0.1300: adapter exclusively used by another host
qeth: Retrying to do IDX activates.
qeth: IDX_ACTIVATE on read channel device 0.0.1300: adapter exclusively used by another host
qeth: Retrying to do IDX activates.
