LVM filter changes are not persistent after reboot

Issue

  • lsblk and blkid show root_lv and swap_lv
  • pvscan does not show physical volume with lvm2 metadata
  • Changes made to global_filter in /etc/lvm/lvm.conf are not persistent between reboots

Environment

  • Red Hat Enterprise Linux 6.6
  • lvm2-2.02.x
  • ext4 filesystem
