RHEL7 Server not booting, I see "dracut-initqueue: warning could not boot."

Solution Verified - Updated -

Issue

As part of our move to RHEL7 have build two RHEL7 servers. One of the servers is sticking at

         Starting File System Check on /dev/mapper/vg0-root...
systemd-fsck[1654]: /sbin/fsck.xfs: XFS file system.
[  OK  ] Started File System Check on /dev/mapper/vg0-root.
dracut-initqueue[1204]: Warning: Could not boot.
dracut-initqueue[1204]: Warning: /dev/vg0/swap does not exist
         Starting Dracut Emergency Shell...
Warning: /dev/vg0/swap does not exist

Generating "/run/initramfs/rdsosreport.txt"

I have re booted this system several times with the same result.

Environment

  • Red Hat Enterprise Linux (RHEL) 7
  • boot hang
