LVM meta data gets overwritten and Volume group is missing sometime after creation

Solution Unverified - Updated -

Issue

  • Volume Group goes missing and the LVM meta data gets overwritten some time after creation.
  • After creation of Physical Volume and Volume group I am able to see them but it gets automatically disappeared after sometime.
  • pvdisplay not listing all the physical volumes.

Environment

  • Red Hat Enterprise Linux Server 6
  • Third Party Cluster
  • HA-LVM Tagging Variant
