Why are my Symmetrix SPC2/SCSI2 LUNs not properly recognised in Red Hat Enterprise Linux?

Problem:
Red Hat Enterprise Linux does not recognize the LUNs presented by an EMC Symmetrix array. I do see a number of LUNs with numbers above 40000, but those have not been defined on the Symmetrix and they are not usable.
In /proc/scsi/scsi, the LUNs are seen as follows:
