pengine repeatedly logs "info: determine_op_status: Operation monitor found resource <resource> active on <node>" in a RHEL 6 or 7 High Availability cluster with pacemaker

Issue

• pacemaker is showing incorrect monitor state for resources which are not online on the node its reporting
• pengine frequently logs that a monitor operation found a resource active on a node, but I've checked and its not active there at all
