squid with NTLM auth stops asking for authentication

Issue

When using squid with NTLM auth after some users connect it stops asking new users for authentication. How can this be fixed?

Environment

  • Red Hat Enterprise Linux 6
  • Squid proxy
  • NTLM user autentication
