Why does creating ext4 filesystem on striped LV crashes server ?

Issue

• Two OCZ Zdrive R4 flash drives are installed on the system. When the devices are a part of a striped LV, the mkfs command crashes the OS. The module ocz10xx.ko for the devices (provided by vendor) is loaded via "insmod ocz10xx.ko".
