EMC PowerPath marks paths and SP as dead in RHEL

Issue

• Storage paths failed and applications or file systems on those devices reported I/O errors
• LUNs are missing and we tried to make LUNs enable all the ways we can but were unsuccessful
• /var/log/messages shows paths being marked as dead:
