Importing Custom Dialogs from EVM 4 to CFME 5

A script has been created to handle importing ManageIQ Enterprise Virtualization Manager (EVM) version 4 provisioning dialogs into the Red Hat CloudForms Management Engine (CFME) version 5 database.
