Where can I find the QLogic iSCSI TOE adapter driver for Red Hat Enterprise Linux 4.4?

This article mainly deals with an HS21 blade which has the Qlogic iSCSI TOE adaptor as an option. Red Hat Enterprise Linux 4 Update 4 does not recognize this adaptor.
