The usb-storage driver does not load automatically for usb cdrom installs on my Dell PowerEdge server with Red hat Enterprise Linux 4. Why not?

Problem
During install of Red Hat Enterprise Linux 4, anaconda fails to load the usb-storage driver from the initrd and the optical CDROM device is not detected. This halts the install process, and the usb driver must be manually loaded to proceed.
