Issue

Creating instances error shows "block device mapping is invalid".
Nova instance creation failure.
Nova boot instance from volume failing.
VolumeNotCreated error, Volume did not finish being created even after we waited error.
Nova appears to hit a timeout when creating boot from volume instances.
