How can I create my own custom banner page when using CUPS in RHEL?

How can I create a custom banner page?
I have created a custom banner page, but the dates aren't printing properly. Question marks (?) are printing instead.
I have created a custom banner page; however the time-at-creation and time-at-processing options only output a ? on the banner page.
