After HPE Smart Adapter (smartqpi) updated firmware from 4.11 to 5.xx, tape and changers duplicated and not working

Issue

  • Firmware update on HPE Smart Adapter 4.11 to 5.00 or later results in duplicate but non-working tape and changer device entries:

    $ lsscsi

  • HPE Smart Controller (smartpqi driver)
    • Tapes and/or media changers attached to the above backplane raid controller
