Storage paths are not reinstated after controller failure in RHEL 5 or 6

Issue

Paths in a multipath map are not reinstated on their own
After a controller failure or reboot on the storage array, devices are not returning to an operational state
See DID_NO_CONNECT SCSI errors and path checker fails after the storage array controller has come back online:

        kernel: sd 0:0:0:36: SCSI error: return code = 0x00010000
        multipathd: sdt: tur checker reports path is down

Environment

Red Hat Enterprise Linux 5 (prior to the 2.6.18-274.18.1.el5 kernel)
Red Hat Enterprise Linux 6 (prior to the 2.6.32-220.el6 kernel)
Storage accessed via fiber channel
