Server crashed after running ql-scan-lun.sh in Red Hat Enterprise Linux 5
Issue
- The system crashed just after running a QLogic-supplied script called "ql-scan-lun.sh". The script ended at 10:00:06 and the calling shell presented a shell prompt. A PowerPath command was executed and found that the system was unresponsive. The error messages seen are:

Oct 6 10:00:06 test kernel: kobject_add failed for 1:0:1:0 with -EEXIST, don't try to register things with the same name in the same directory.
