Server crashed after running ql-scan-lun.sh in Red Hat Enterprise Linux 5

Solution Unverified - Updated -

Issue

  • The system crashed just after running a QLogic-supplied script called "ql-scan-lun.sh".  The script ended at 10:00:06 and the calling shell presented a shell prompt.  A PowerPath command was executed and found that the system was unresponsive. The error messages seen are:

Oct  6 10:00:05 test kernel: error 1
Oct  6 10:00:05 test kernel: scsi 1:0:1:0: Unexpected response from lun 0 while scanning, scan aborted
Oct  6 10:00:06 test kernel:   Vendor: DGC       Model: RAID 5            Rev: 0430
Oct  6 10:00:06 test kernel:   Type:   Direct-Access                      ANSI SCSI revision: 04
Oct  6 10:00:06 test kernel: kobject_add failed for 1:0:1:0 with -EEXIST, don't try to register things with the same name in the same directory.
