Why installation stuck with error "isolinux: Disk error 80, AX = 426C, drive 9F" ?

Issue

  • Attempting to build new server from satellite and recieved the following error : 
        loading vmlinuz.....
        loading initrd.img......
        isolinux: Disk error 80, AX = 426C, drive 9F

Environment

  • Red Hat Network Satellite or Proxy 5
