Why am I getting BadUsernameSessionEvents in CloudForms 2.0

Issue

We see connection errors to the work server from the EVM appliance. BadUsernameSessionEvent is logged to evm.log periodically. Could not locate the orgin (screen in EVM) to fix that error.
